Forms & Waivers

Please download and print either the New Member or Renew Member application.   The Waiver/Release form also needs to be signed and returned in order to be able to use the range.  Once filled out, please mail the forms with a check to the address on the form.  Waivers are attached to the applications, and the waiver is also listed below, separately.  Note:  These are not fillable forms.  They have to be downloaded and printed.  If you have difficulty downloading the pdf forms, you may also request them by email via info@SarasotaArchers.org.  Thank you.
