Forms & Waivers

Please download and print either the New Member or Renewal application.   The waiver form also needs to be signed and returned in order to be able to use the range.  Once filled out, please mail the forms with a check to the address on the form.  Waivers are attached to the applications, and the waiver is also listed below, separately.  Note:  These are not fillable forms.  They have to be downloaded and printed.  If you have difficulty downloading the pdf forms, you may also request them by email via info@SarasotaArchers.org.  Thank you.
